Center for Nursing and Rehabilitation (CNR) has developed a specialized program that provides a comprehensive, holistic approach to HIV/AIDS care. Our experienced team of nurses, therapists, social workers, and nutritionists works closely with each patient’s primary care physician to create an individualized plan of treatment that best meets his or her needs and lifestyle.

Our HIV/AIDS Home Care Program is the largest in New York State, allowing CNR patients to have access to a wide range of continuing healthcare services such as rehabilitative care, social work services and respiratory therapy. The AIDS Drug Assistance Program (ADAP), in conjunction with Medicaid, allows us to provide home care for individuals who would otherwise have been ineligible.

HIV/AIDS Home Care Patient Eligibility Requirements

To be eligible for participation in our program, you must:

Be at least 21 years old

Have an HIV/AIDS diagnosis

Individuals with HIV/AIDS that have the AIDS Drug Assistance Program must be Medicaid eligible or able to pay privately

Be a resident of the Bronx, Brooklyn, Manhattan or Queens

Require in-home preventive, diagnostic and supportive services to improve, restore and/or maintain functioning

Be eligible for placement in a skilled nursing or health-related facility

Have a primary care physician in the community

Have a physician determine that the individual’s health-care and related psychosocial needs can be met at home

Have a safe home environment
